FREE Senior Services for Seminole County residents that meet financial and age eligibility.
Advance Care Planning is an important step toward receiving the care that meets your long term needs and preferences. Our special project is for a limited time. Persons that qualify will meet with an attorney to discuss and prepare Advance Directives, which include, “Living Will”, “Designation of Healthcare Surrogate”, and “Durable Power of Attorney”.
Living Will: A document that lets you state your wishes for end-of-life medical care, in case you become unable to communicate your decisions.
Designation of Healthcare Surrogate: A document that allows you to appoint a person to make healthcare decisions for you at a time you designate.
Durable Power of Attorney: A document that enables a person, (a trusted relative or friend) to handle specific legal and financial responsibilities.
We are holding the Advance Directives monthly event at the Winter Springs Senior Center. If you are unable to attend one of these events, please contact our office and we can schedule you an appointment to come directly to our office.
